• I have read the information statement for this study, and I agree to participate.
• I am the mother of a baby aged 23 months or younger, aged 18 years or over and currently living in Australia or New Zealand.
• I understand that my information will be stored confidentially and that the study personnel will not reveal my identity.
• I understand that I can choose to withdraw from this study at any time without any negative consequences by contacting the researchers and informing them of my withdrawal. I understand that if I withdraw, I can request that all data collected up to that point be destroyed and not included in the study.
• I have been provided with details of support services that I can contact in the event that I experience an emotional reaction to participating in this study.
